By way of a close analysis of the B-Deduction, Adam Dickerson makes the distinctive claim that the Deduction is crucially concerned with the problem of making intelligible the unity possessed by complex representations - a problem that is the representationalist parallel of the semantic problem of the unity of the proposition. Our first reference to the name comes from Demosthenes, who in his second Olynthiac tries unconvincingly to disparage the fighting qualities of Philip's mercenaries andDemosthenes adds no explanation, and it was left to commentators and lexicographers to unearth a relevant fragment from thePhilippicaof Anaximenes of Lampsacus.

Background: As part of an ongoing Scandinavian project on the dignity of care for older people, this study is based on ‘clinical caring science’ as a scientific discipline. Clinical caring science examines how ground concepts, axioms and theories are expressed in different clinical contexts. Central notions are caring culture, dignity, at-home-ness, the little extra, non-caring cultures versus caring cultures and ethical context – and climate. Aim and assumptions: This study investigates the individual variations of caring cultures in relation to dignity (...) and how it is expressed in caring acts and ethical contexts. Three assumptions are formulated: (1) the caring culture of nursing homes influences whether dignified care is provided, (2) an ethos that is reflected on and appropriated by the caregiver mirrors itself in ethical caring acts and as artful caring in an ethical context and (3) caring culture is assumed to be a more ontological or universal concept than, for example, an ethical context or ethical person-to-person acts. Research design: The methodological approach is hermeneutic. The data consist of 28 interviews with relatives of older persons from Norway, Denmark and Sweden. Ethical considerations: The principles of voluntariness, confidentiality and anonymity were respected during the whole research process. Findings: Three patterns were revealed: dignity as at-home-ness, dignity as the little extra and non-dignifying ethical context. Discussion: Caring communion, invitation, at-home-ness and ‘the little extra’ are expressions of ethical contexts and caring acts in a caring culture. A non-caring culture may not consider the dignity of its residents and may be represented by routinized care that values organizational efficiency and instrumentalism rather than an individual’s dignity and self-worth. Conclusion: An ethos must be integrated in both the organization and in the individual caregiver in order to be expressed in caring acts and in an ethical context that supports these caring acts. One of the more intriguing figures of the first period of the Successors is Nicanor, the lieutenant and admiral of Cassander. He came into prominence when he assumed command of the Macedonian garrison at Athens, late in 319 B.c. After distinguishing himself there he took a fleet to the Bosporus, where with Antigonus' collaboration he won a decisive victory over Polyperchon's royal navy. Subsequently his aspirations became sufficiently lofty to threaten his patron's security, and Cassander took elaborate precautions to ensure (...) his arrest and condemnation. Nicanor was clearly a figure of considerable importance; yet no source even hints at his origins and family background. Can conjecture go any way to filling the gaps? (shrink)

Bronchiolitis obliterans is the most common pathologic finding on biopsy, characterized by fibrous granulation tissue, which obliterates the lumen of the bronchiole. Clinically, in the absence of tissue for pathology, BO syndrome refers to a progressive irreversible drop in the forced (...) expiratory volume in 1 second. Recently, a broader definition of chronic rejection, termed "chronic lung allograft dysfunction", has been used to encompass a more inclusive definition of posttransplant dysfunction. Recently, the lung transplant community has come to realize that chronic rejection may be the final common result after repetitive epithelial insults. Acute rejection, infection, and alloreactivity to mismatched HLA antigens are a few of these insults that damage the surface of the bronchioles. Recent evidence of autoimmunity to the normally hidden structural proteins collagen V and K-α1 tubulin have been correlated with a BO phenotype as well, perhaps correlating the epithelial damage with a mechanism for developing BO lesions. Many immunomodulatory medications and treatments have been studied for effectiveness for the treatment of chronic lung allograft dysfunction. New drugs, which more precisely target the immune system, are being developed and tested. Further study is required, but recent advances have improved our understanding of the pathogenesis and potential intervention for this common and deadly complication of lung transplantation.Keywords: lung transplant, bronchiolitis obliterans syndrome, rejection. Objectives: to assess physicians’ and patients’ views in Saudi Arabia towards involving the patient versus the family in the process of diagnosis disclosure and decision-making, and to compare them with views from the USA and Japan.Design: A self-completion questionnaire was translated to Arabic and validated.Participants: Physicians from different specialties and ranks and patients in a hospital or attending outpatient clinics from 6 different regions in KSA.Results: In the case of a patient with incurable cancer, 67% of doctors and 51% of (...) patients indicated that they would inform the patient in preference to the family of the diagnosis . Assuming the family already knew, 56% of doctors and 49% of patients would tell the patient even if family objected . However, in the case of HIV infection, 59% of physicians and 81% of patients would inform the family about HIV status without the patient’s consent . With regards to withholding ventilatory support, about 50% of doctors and over 60% of patients supported the use of mechanical ventilation in a patient with advanced cancer, regardless of the wishes of the patient or the family. Finally, the majority of doctors and patients were against assisted suicide.Conclusions: Although there was more recognition for a patient’s autonomy amongst physicians, most patients preferred a family centred model of care. Views towards information disclosure were midway between those of the USA and Japan. Distinctively, however, decisions regarding life prolonging therapy and assisted suicide were not influenced to a great extent by wishes of the patient or family, but more likely by religious beliefs. The liberal and democratic political order is underpinned by universal principles of justice. However, the universality of these principles is now being questioned and undermined by challenges from postmodernism, communitarianism, multiculturalism and other forms of anti-foundationalism. These challenges highlight the sheer diversity of cultures and values, treating liberal values and democratic political culture as one idea of social organization amongst many. While social and political orders are capable of almost endless variation, it may be that not every diverse order is (...) legitimate and we can justifiably regard some forms of social organization as beyond the pale and a 'thin' form of universalism has been developed to support this contention. The core idea behind 'thin universalism' is that it is possible to rehabilitate the universalist aspirations of normative theory while acknowledging the force of many antifoundationalist and multiculturalist criticisms of universalism. This book brings together prominent international political theorists to explain and discuss this contested concept and explore how we can stand up for and act upon principles in a world characterized by cultural difference, moral skepticism, and political division. The authors present a variety of perspectives: some chapters are sympathetic to the idea of a thin universalist account of justification, some are reluctant advocates and others are skeptical about its success or even desirability. Opening out core debates in contemporary normative theory, this volume will be of great interest to researchers and scholars working in the fields of contemporary political theory, political philosophy and IR theory. Dirac's equation is reviewed and found to be based on nonrelativistic ideas of probability. A 4-space formulation is proposed that is completely Lorentzinvariant, using probability distributions in space-time with the particle's proper time as a parameter for the evolution of the wave function. This leads to a new wave equation which implies that the proper mass of a particle is an observable, and is sharp only in stationary states. The model has a built-in arrow of time, which is associated with (...) a restriction to positive-energy solutions. The usual solution for a Coulomb field is retained, though it now implies a slightly different charge distribution. The conventional nonstationary solutions become invalid. The new formulation appears to offer a resolution of difficulties that have been associated with Dirac's equation. It also predicts the occurrence of virtual pairs at a level that may be experimentally testable, and suggests a mechanism for self-cancellation of the vacuum energy. (shrink)

Strawson's book Individuals is subtitled An Essay in Descriptive Metaphysics. ‘Descriptive metaphysics’, he writes, ‘is content to describe the actual structure of our thought about the world’, whereas ‘revisionary metaphysics is concerned to produce a better structure’; it is distinguished from logical or conceptual analysis in scope and generality, rather than in fundamental intention. The book is divided into two parts; in Strawson's words, ‘the first part aims at establishing the central position which material bodies and persons (...) occupy among particulars in general. … In the second part of the book the aim is to establish and explain the connexion between the idea of a particular in general and that of an object of reference or logical subject.’.
